Who decides
The competent labour inspector of the State Labour Inspectorate (Państwowa Inspekcja Pracy). Appeals go to the district labour inspector (PIP).
Permit or licence
A child under 16 may do paid work only for an entity running cultural, artistic, sports or advertising activity. It needs prior consent from the legal representative or guardian and a permit from the labour inspector (Kodeks pracy, art. 304(5) § 1). The entity applies (§ 2). The application must include: written consent of the legal representative or guardian; an opinion from a psychological and educational counselling centre; a doctor's certificate of no contraindications; and, if the child must attend school, an opinion from the school head (§ 4). The inspector refuses if the work endangers the child's life, health or development, or school attendance (§ 3). The permit is an administrative decision (PIP). The legal representative can ask the inspector to withdraw it (§ 6). No lead time is stated in the sources checked.

Working hours
No fixed limits in the Code for this work. The permit must state the allowed period of work and the allowed daily working time (art. 304(5) § 5). PIP reports that inspectors required applicants to respect the daily working time set in the decision.
School
The school head's opinion on whether the child can meet school obligations is required (§ 4). The permit is refused if the work threatens school attendance (§ 3).
Chaperone and supervision
Not set in the Code. The permit may include other arrangements needed for the child's welfare (§ 5). PIP reports that inspectors required applicants to provide adult care for the children during rehearsals and performances.
Earnings
Not stated in the sources checked.
